Add accurate Kannons & Katapults key generator

Add knkgen to reproduce the registration calculation recovered from the QuickBASIC 3 executable.

Match QBSERIAL name normalization, x87 extended-precision square roots, QuickBASIC INT behavior, and the original BBS/sysop field ordering.

Also account for DOSBox 0.74's premature truncation of 80-bit operands. Generate and bit-verify a compatible decimal spelling when it differs from
the standard x87/MSEM87 registration value.
